Job Description: A Health Care Occupational and Case Manager is responsible for coordinating and managing the care of patients with complex medical conditions or disabilities. Health Care Occupational and Case Managers should have a background in nursing, social work, or a related field, along with experience in case management. They should have strong communication, organizational, and problem-solving skills. A compassionate and empathetic approach to patient care is essential. Depending on the specific role, certification as a Case Manager (CCM) or in a related field may be required or preferred.
