Overview:

The Peer Recovery Coach provides non-clinical services designed to support patients in establishing and sustaining recovery from high-risk substance use disorders. Working collaboratively as part of the hospital care team, the Peer Recovery Coach offers peer support, encouragement, and motivation to help patients explore behavior change related to substance use and engage in recovery-oriented services.

This role includes direct interventions with patients during hospitalization and within the community to facilitate access to treatment programs, recovery resources, and supportive services. The Peer Recovery Coach will engage with patients in the Emergency Department, assigned inpatient units, and through community outreach efforts following hospital discharge to promote continuity of care and reduce barriers to recovery.

Responsibilities include conducting outreach and engagement activities, developing individualized service plans, coordinating referrals, and promoting successful linkage to treatment and recovery support services. The Peer Recovery Coach serves as a trusted resource and advocate, leveraging lived experience and recovery principles to foster hope, resilience, and patient-centered support.

This position is grant funded and is contingent upon continued grant funding availability. Employment in this role may be limited to the duration and terms of the grant award.

Responsibilities:
  • Works with Behavioral Health nursing team to identify patient’s history of substance abuse disorder treatment history, motivation to change behavior and other needs for community support services.
  • Identifies patient’s readiness to change and uses motivational interviewing techniques and other communication skills.
  • Works with hospital team members to identify community support services aligned with patient plan. Works with other health providers and community resources collaboratively to support patient needs.
  • Assists patient and family with understanding how to utilize community services.
  • Reduces barriers for patients to access care including coordination of services such as securing appointments and transportation.
  • Acts as a peer support throughout the hospitalization and post-discharge, as appropriate to facilitate attainment of goals. Provides family members with recovery support materials. Assists patients in setting personal recovery goals.
  • Provides education on risks of overdose and assures that patient and significant others have naloxone and are trained to use.
  • Models coping techniques and self-help strategies.
  • Other duties as assigned. 
Qualifications:
  • PEER must be in recovery for at least 18 months.
  • BLS certification within 90 days of hire.
  • Mental Health First Aid course within 6 months of hire.
  • PEER certification within 2 years of hire.

Education:
  • High school graduate or equivalent
